Insurance Certificate Checklist
What your agent should provide us with on their
certificate of insurance or Acord form (if available).
First Western Bank & Trust, DBA: Advance Acceptance, ATIMA, 100 Prairie Center Drive, Eden Prairie, MN 55344 as certificate holder, loss payee or additional insured, depending on your finance or lease contract.
Contract (agreement) Number
This is located on your finance documents from Advance Acceptance on the first page.
Cost of Equipment
The total cost of the equipment is typically on the dealer's invoice. This is the cost of the actual equipment, not the total amount of the loan.
The effective and expiration dates of the policy. Just like your homeowner or auto insurance.
The name of your insurance provider (or carrier) and your policy number.

When does the insurance certificate need to be in to Advance Acceptance?
We prefer that the certificate be sent in with all other documents sent to the account manager. If that isn't possible, as soon as possible thereafter, to make sure that customers are protected from loss or damage.
Will Advance Acceptance contact the agent?
We will follow-up with the agent to obtain the correct documents, but the customer must make an initial contact with their agent to add the equipment to their policy.
